Occupational Therapy Assessments

Some OT assessments are funded under NDIS and My Aged Care packages. See details below.

NDIS Functional Assessments

Functional assessments are designed to measure the cognitive, physical and every day capabilities of individuals with disabilities funded by the National Disability Insurance Scheme.  We work together to inform family and carers about what their loved one is and isn't capable of doing, as well as what services are necessary to allow you to stay independent in daily living. It also helps us set your  therapy goals.
 
By completing a number of tasks, we'll determine your functional capacity when performing day-to-day activities, and any challenges or gaps that need addressing. The assessment markers include mobility and transfers, functional cognition, self care and self management, social and community interaction, and engagement in life.

Activities of Daily Living Assessments (ADL)

This practical ADL assessment takes place at your home, or in your aged care or rehabilitation setting, and measures your ability to engage in meaningful life activities. such as dressing or eating and more complex tasks like managing accounts.
 
Our aim is to identify any issues or gaps in your care for example:

  • safety issues

  • obstacles to independence

  • opportunities to improve your quality of life

 
Afterwards I'll produce a detailed report with recommendations for necessary or additional home care assistance, rehabilitation or the relevant scheme support. If needed, I can send the report and supporting documentation for Home Care Packages or aged care services to the related organisation too.

The results will help shape your therapy goals and if assistance equipment is needed, we can visit local suppliers to trial options.

Functional Cognitive Assessments

This assessment, based on the Allens Cognitive Disability Model, focuses on functional cognition and how brain injuries or conditions impact people’s ability to manage necessary tasks.
 
It involves a comprehensive analysis of a person's capability, provides a whole-person perspective, taking into account physical, mental, emotional, social, environmental, cultural, spiritual and occupational factors.
 
The results reveal what a person “can do, will do and may , and do” are helpful for family and Carer’s to understand what their loved one is capable of doing or not at a particular stage of cognitive change.

I'm currently working with The Do Network to deliver their Introductory Functional Cognition Workshop to train other occupational therapists in the process, so this is a great opportunity to take advantage of my expertise in this area.

Government funding assistance

NDIS

OT services can funded under an NDIS plan via:

  • Capacity Building: Improved Daily Living

  • Capital Supports: Home Modifications and Equipment

  • No referral required

Seniors needing support to live independently at home can access funding for OT services via:

  • Commonwealth Home Support Programme

  • Home Care Packages (HCP) Levels 1–4

  • A referral is required
